MACHINE MAINTENANCE REPORT
– WASHER & DRYER

When a machine experiences a technical issue, the Attendant on duty must complete the form below. Please provide as much information as possible and click SUBMIT. The form will be automatically sent to management via email.

If the machine is unable to be used properly/safely:

1. Place the Deactivation Card on the washer/dryer reader to deactivate it.
   Verify that the reader shows DEACTIVATED before proceeding.

2. Place Out of Order signage on the machine(s).

TO REMOVE CLOTHING / OPEN A WASHER DOOR DUE
TO WASHER MALFUNCTION ONLY:

• Turn off breakers for the required machine(s) — TWO switches per machine.

  -- Wascomat model (white): takes 1–5 minutes to reset/unlock door
  -- Huebsch model (silver): takes 10 minutes to reset/unlock door.
